NIAGARA FC OPDL COACH APPLICATION AND TEAM STAFF REGISTRATION

Welcome!  Thank you for your interest in applying for

NIAGARA FC's OPDL Teams!

This form will be used to help us to evaluate, assess, and if brought on board, upload you onto your teams and create a database of your credentials!

This form is also for returning Coaches/Bench staff and will be used to keep your files current and allow us to add you to your respective team.

Please ensure that you have read the NIAGARA FC OPDL HEAD COACH Job Description provided on the next page and take the time to go through each page carefully.  

Please have your NCCP# ready, and any Certification dates and documentation available for upload.

• Your NCCP # - this can be found on coach.ca in your locker.  If you have done Coaching Courses through Ontario Soccer/Canada Soccer, you should have a login/password.  This will help to provide us with a good knowledge of your current Coaching qualifications.
• Documents of proof of Certification - This may be requested upon acceptance
• Proof of your Police Check - either a receipt of your submission or the actual approved document.
• Proof of Standard CPR with AED
