NFSC - NIAGARA FC's OPDL Coach Positions and Bench Staff

This form will be used to help NFSC Technical Staff and Admin to evaluate, assess, and, if brought on board, upload you onto your respective team, and to create and maintain a database of your credentials!

This form is also for returning Coaches/Bench staff and will be used to update your records, and to add you to your respective team/s for the upcoming season.

Please ensure that you have read the NFSC - NIAGARA FC OPDL HEAD COACH Job Description provided on the next page, and please take the time to go through each page carefully.  

Have your NCCP# ready, and any Certification dates and documentation/certificates available for upload.

• Your NCCP # - this can be found on coach.ca in your locker.  If you have done Coaching Courses through Ontario Soccer/Canada Soccer, you should have a login/password.  This will help to provide us with a good knowledge of your current Coaching qualifications and to help us verify them.
• Documents of proof of Certification to upload.
• Proof of your Police Check - either a receipt of your submission or the actual approved document.
• Proof of Standard CPR with AED
